Medicare Special Needs Plans (SNP)

In certain situations a person who receives medicare benefits may qualify for additional benefits.

The following individuals may qualify:

- Have a Chronic Health condition(Chronic SNP)

Individuals who have certain chronic health conditions such as; cardio vascular disease, diabetes, COPD(chronic obstructive pulmonary disease), CHF(congestive heart failure) and other chronic health conditions defined by Medicare.

- Have both Medicare and Medicaid (Dual Eligible SNP)

Individuals who are entitle to both Medicare and Medicaid benefits.

- Living in an Institution (Institutional SNP)

Individuals who live in institutions such as nursing homes or individuals who live in a long-term care skilled nursing facility (LTC SNF), intermediate care facility, or assisted living facility.

Medicare and/or the insurance company you choose will have specific requirements you must meet in order to enter into the plan you choose.

For example: you may be required to get proof of your condition from you health care provider in order to enroll in a Chronic SNP, or Some Dual Eligible SNP's require verification of your Medicaid level/status.
